Our CC4C Financial Medical Assistance is closed. We have exhausted all funds.
Financial medical reimbursement applications will reopen on August 1, 2024.

Navigating life with a rare or undiagnosed medical condition is expensive. Many treatments, therapies, medications and equipment are not covered by insurance. To help ease the financial burden for families, CC4C provides financial medical assistance to reimburse families for eligible expenses (see below for complete list).

Applications are reviewed as they are submitted and awarded on a funds-availability basis. We receive more requests than we're able to fund, so to increase chances of funding please read the eligibility criteria carefully and submit a complete application.

If you have participated in our Dream Teams program, you will be able to access your application through your portal via the CC4C case management system.

  • Your application includes household information and category reimbursements all in ONE application

  • There is NO acceptance waiting period prior to applying for financial assistance

  • When applying through the application link above YOU MUST PROVIDE ALL RELEVANT INFORMATION AND UPLOAD VALID RECEIPTS before submitting

  • Once an application is submitted, no edits will be allowed

  • You will receive the status of your application within 2 weeks

Reimbursements are for medical expenses that occur between August 1, 2023 through June 30, 2024

  • Medical appointments

  • Travel expenses for medical visits (lodging & air travel only) 

  • Medical supplies

  • Dental

  • Genetic Testing

  • Adaptive equipment

  • Assistive technology

  • Accessibility modifications and equipment

  • Assistive animal training

  • Mental health services 

  • Adaptive camps, clinics and experiences

  • Patient health record management services

  • Respite care

  • Therapies: Acupuncture, equine, in-home, etc.

  • Tutoring/education for specific disabilities: ex: Braille 

  • Chiropractic care 

  • Medications 

  • Supplements: prescribed by practitioner with official letterhead

  • Mental health for parents & siblings (up to 30% of allotment allowed)
