 ASSISTANCE 

Navigating life with a rare or undiagnosed medical condition is expensive. Many treatments, therapies, medications and equipment are not covered by insurance. To help ease the financial burden for families, CC4C provides financial assistance for expenses such as: 

 

  • Medical visits  

  • Medical supplies 

  • Dental 

  • Adaptive equipment 

  • Assistive technology 

  • Accessibility modifications and equipment 

  • Service dog training  

  • Mental health services  

  • Patient health record management services 

  • Respite care 

  • Therapies: Acupuncture, equine, in-home, etc. 

  • Potty training needs

  • Tutoring/education for specific disabilities: ex: Braille  

  • Chiropractic care  

  • Medications  

  • Supplements: prescribed by practitioner with official letterhead for line item - Adaptive camps, clinics and experiences 

  • Travel (airline, hotel, gas) to medical facilities/appointments 

​CC4C Champion and Sustaining Families receive a reimbursement allotment each year.  To broaden our reach and help families across the state, twice a year we open applications to our Statewide Medical Financial Assistance Fund.

Statewide Medical Financial Assistance applications will open in 2022 - dates to be announced.
